The three non-negotiable ingredient classes in a barrier repair moisturizer are ceramides, cholesterol, and free fatty acids — specifically because those three lipids make up the lamellar matrix in healthy stratum corneum. When the barrier breaks down, it is almost always because that lipid ratio has been disrupted, so a well-formulated repair cream attempts to replenish what is missing.

Ceramides are sphingolipids. On a label, look for ceramide NP, ceramide AP, ceramide EOP, ceramide NS, or older naming conventions like ceramide 1, 3, or 6-II. Phytosphingosine and sphingosine are precursors that encourage the skin to synthesise its own ceramides — useful, but slower-acting. A formula with multiple ceramide types is more effective than a single-ceramide product because healthy skin contains at least nine distinct ceramide subtypes working in concert.

Cholesterol is frequently omitted even from products marketed as barrier repair, because it adds raw material cost and requires careful formulation to keep aesthetics acceptable. Its absence is one of the most common weaknesses in otherwise-decent ceramide products. If cholesterol appears on the label — ideally before the fragrance, indicating a meaningful concentration — it is a strong signal that the formula was built with the dermatology literature in mind.

Free fatty acids complete the trio. Linoleic acid (omega-6) is particularly important because barrier-compromised and acne-prone skin consistently show relative linoleic acid deficiency. It can appear as linoleic acid directly, or delivered via linoleic-rich oils such as rosehip, safflower, or evening primrose. Palmitic and stearic acid also appear in well-structured formulas.

Beyond those three, occlusives like petrolatum, mineral oil, and dimethicone slow transepidermal water loss (TEWL) while the lipid matrix rebuilds — they are genuinely useful but do not repair on their own. Humectants like glycerin and hyaluronic acid pull water into the epidermis so there is something worth sealing in.

Ingredients to treat with scepticism: botanical extracts and peptides positioned after the preservatives on the label are present in concentrations too small to do measurable work. Fragrance — natural or synthetic — is the leading cause of contact sensitisation in leave-on skincare and is the ingredient most worth avoiding on a compromised barrier. A product with no ceramides, no cholesterol, and prominent fragrance is a moisturizer, not a barrier repair formula, regardless of its marketing language.

In practical terms: look at the first eight ingredients after water. If ceramides and cholesterol appear in that zone, the product is likely functional. If they appear only after the fragrance and preservatives, they are present as label claims rather than active doses. The label position rule is the single most useful tool a shopper can apply without any formulation background.
